Instructions for filling out form:

• We do not accept cash
• A cheque or money order, or bank draft payable to Quinndale Community Council,
• NSF cheques are subject to a $ 10 Fee.
• No post dated cheques accepted
• First time registrants must provide a copy of a Birth Certificate or valid proof of age document
• Separate registration forms and separate payments must be submitted for each player.
• Completed registration forms will be processed in order of receipt. Quinndale is not responsible for undelivered mail.  
• No guarantees with registrations received where the cheque does not clear. You will be contacted and notified, but paid registrations will take priority.
